1: Certificate of Origin:
- A stamped self-addressed return envelope.
- A copy of all original documents
Fee: $10 money order for Certificate of Origin with one attached invoice. $20 money order for certificate of origin with two or more attached invoices.
2: Commercial Invoice:
- A stamped, self-addressed return envelope.
- A copy of all original documents.
Fee: $4 per $1000.00 rounded to the nearest 1000. The minimum fee is $15.00, and the maximum fees are $1250.00.
3: Commercial Documents (Agreements):
- The agreement must be signed and notarized in front of a notary public and include the following statement on the same page signed by the applicant and the two witnesses: “Before me appeared personally Mr/Mrs/Ms…….. and witnesses Mr/Mrs/Ms……… and signed this document, this day …/…./….”.
- The document must be authenticated by the Secretary of State. (please include link).
- A stamped, self-addressed return envelope.
- A copy of all original documents.
Fee: A money order in the amount of $75 per document payable to the Consulate General of Lebanon in Los Angeles.
